A useful symptom record connects medicine timing to a concrete change in health or function. It should take minutes, not become a burden.

Record one line per change
Write the dose time, symptom start time, body location, severity, duration, and what you could not do normally. Add fever, glucose, stool count, fluid intake, rash photos, or urine changes only when relevant.
Separate infection from possible side effects
Track whether the original infection is improving, unchanged, or worse. This helps the clinician evaluate both treatment effectiveness and medication safety.
Call before the diary is complete
Emergency symptoms require emergency services. New tendon pain or peripheral-neuropathy symptoms require prompt prescriber contact under the Medication Guide.
Bring context
Attach a current medicine and supplement list and note strenuous activity, sun exposure, missed doses, vomiting, or reduced food and fluid intake.
